- Preferred date (within the next 2 weeks)
- Preferred time slot
- Your address
- Payment method (Venmo, Cash App, or Cash)
- Optional: referral name for 15%+ discount
- Optional: trim coat add-on upgrade

We'll review your request and send a confirmation email once accepted.
